Below are the registration forms for the 2007 - 2008 hockey season. Please make sure you have the latest version of Adobe Acrobat reader installed on your computer. We have provided a link to download Adobe on the main web page. To download the form just right click the icon with your mouse and select "Save target As". Please download the LHC registration instructions document first. This document will explain what forms need to be completed and where to send them. Lastly, We have included the new Liberty Ice Hockey Club logo for download. Just right click the logo icon and download to your computer. You can use the logo to sport your Liberty Ice Hockey team spirit. Use it as a background image on your computer.
Please note: We have added the clothing order form on this page. This form is in Microsoft Excel format. In order to complete the form and e-mail to Tom Kleckner you must have Microsoft Excel installed on your PC or MAC.
